Dementia & specialist care at home

Dementia care and support for more complex needs at home, built around familiar routines, what has changed and what matters to the person.

Familiar moments matter.

Dementia care at home

A familiar routine. A personal approach.

We get to know the person beyond the care they need: the routines they value, how they like to communicate, and who they want involved in decisions.

Enquiring for a family member? Start with what you have noticed. No care plan is needed first.

The things that feel like home

Familiar things, familiar comfort.

A favourite teapot, a framed landscape, a clock that has always sat on the shelf, the record that gets everyone humming.

Everyday objects and music say a great deal about a person’s life. We learn which belongings, songs and small habits matter, so conversations and routines start from what is already familiar.

Personal preference and company, not a treatment.

Fresh air and favourite places

A walk in the garden.

For many people, time outdoors is part of an ordinary day: a coat on its usual peg, a few flowers cut for the table, a seat by the path.

If the garden or a regular walk matters, we build time outdoors into the day in a way that feels comfortable and safe.

Any safety arrangements will be agreed with you first.

Things you may want to talk about

Daily routines

Mealtimes, getting ready and the familiar parts of the day to keep.

Communication and choice

How the person likes to be addressed and who they want involved.

Support already in place

Who helps now, and where the gaps are.

Before you decide

Is this the right support?

“Specialist care” can mean different things to different families, so we start from the practical support needed rather than a service name.

In our first conversation, we talk through:

  • The needs we can support at home
  • The skills and experience of the carers who would visit
  • Anything outside our service, and other support that may help

The short callback form is for contact details only. Keep personal health information for a conversation with the team.

Your questions may span more than one service.

Everyday help and overnight support can be part of the same enquiry. We look at the whole situation, so there is no need to choose just one page or category.
